The Philippines appeals to Indian students for clear reasons: teaching and daily life in English, a tropical climate and diseases similar to India’s. But its American-style system, a pre-medical bachelor’s degree followed by a four-year MD, has caused real confusion about India’s eligibility rules. Here is how it works now.
Philippines at a glance
| Structure | Pre-medical BS degree, then MD program, then internship |
|---|---|
| Entrance test | NMAT (National Medical Admission Test) before the MD |
| Tuition (indicative) | About ₹4-7 lakh a year; ₹25-45 lakh overall |
| Language | English in teaching and widely in daily life |
| Climate | Tropical |
| NMC position | MD must be 54 months excluding the BS, with a 12-month internship and other FMGL conditions |

How the Philippine system works
- Pre-medical Bachelor of Science (BS): students coming from Class 12 first complete a BS degree, often in biology or a related subject. Its length depends on the course and the student’s prior education.
- NMAT: before entering the MD, students take the National Medical Admission Test.
- Doctor of Medicine (MD): a four-year program with clinical clerkship.
- Internship: a period of post-graduate internship.
NMC eligibility: what changed
When FMGL 2021 set a minimum of 54 months for the medical course, many Philippine pathways did not fit, because neither the BS nor the four-year MD alone met it. NMC also stated that the BS course cannot be counted as part of the MBBS-equivalent course.
- In December 2023, NMC gave a one-time relief to students who were already enrolled in the BS course or physically studying there when the regulations were published, with an additional year of internship in India under the CRMI rules.
- In 2025, the Philippines’ Commission on Higher Education (CHED) confirmed to the Indian Embassy in Manila that its MD programs run 54 months, not counting the pre-medical course, are taught in English, cover the required subjects and include a one-year internship, in line with NMC conditions.
The practical message: choose an MD program that is at least 54 months on its own, with a 12-month internship at the same institution or its affiliated hospital in the Philippines. Count the BS separately. Ask the university for written confirmation, and check the latest NMC and Indian Embassy advisories before paying. See FMGL 2021 explained.
Total cost and duration
Because the BS comes first, the total time from Class 12 to graduation is usually longer than a six-year program elsewhere. Add the FMGE and the 12-month internship in India afterwards. Indicative tuition is around ₹4-7 lakh a year, about ₹25-45 lakh overall, with living costs that are moderate outside Manila. Use the total cost calculator with the full number of years.
Pros and cons
| Pros | Cons |
|---|---|
| English everywhere: no new language for patients | Longer total duration (BS + MD + internship) |
| Disease patterns similar to India | Eligibility history has been confusing; documents matter |
| Warm climate, rice-based food | Typhoon season and natural hazards |
| US-style training useful for USMLE aspirants | Quality varies between schools |
Checks before admission
- MD program duration on its own (at least 54 months) and internship arrangement, in writing.
- CHED recognition of both the BS and the MD programs.
- NMAT requirement and the score the school accepts.
- Tuition paid directly to the university, against official receipts.
- Documents apostilled as required by the Philippine Department of Foreign Affairs.
For travel, compare flight prices from Indian cities to Manila early, and set up an Airalo eSIM for arrival. For tuition payments in USD or PHP, compare your bank with Wise and other regulated providers.
