Some MBBS abroad students hope to practise in the United States or United Kingdom rather than, or as well as, India. Both routes are open to international medical graduates, but both are long, expensive and competitive. Here is what each involves.
USA: the USMLE route
- Check school eligibility. Your medical school must be listed in the World Directory of Medical Schools with an ECFMG note showing graduates can apply. ECFMG also has an accreditation policy linking eligibility to recognised accrediting agencies; check its current status for your country on ecfmg.org.
- USMLE Step 1 (basic sciences, pass/fail) and Step 2 CK (clinical knowledge, scored).
- English proficiency through the test ECFMG accepts (currently OET Medicine).
- ECFMG certification once the requirements and your degree are verified.
- Residency match through the National Resident Matching Program. US clinical experience, letters of recommendation and Step 2 CK scores matter a lot.
- Step 3 usually during residency, then state licensing.
The USMLE route typically takes several years and significant money for exams, applications and US clinical experience. Strong Step 2 CK scores and early planning are key. Resources such as Lecturio’s USMLE video lectures and question banks are popular with international students for Step 1 foundations.
UK: the PLAB route
- Acceptable primary medical qualification: your degree must be accepted by the General Medical Council (GMC). Check on the GMC website.
- English language: usually IELTS Academic with an overall 7.5 and at least 7.0 in each part, or OET (Medicine) with grade B in each part.
- PLAB 1: a written exam of single-best-answer questions, held in the UK and some other countries.
- PLAB 2: an objective structured clinical exam (OSCE), held in the UK.
- GMC registration with a licence to practise, then applying for jobs, often starting with Foundation-level or trust grade posts.
PLAB content is aligned with the UK’s Medical Licensing Assessment. The route is generally faster than the US route, but UK training posts are competitive.
Side-by-side comparison
| USA (USMLE) | UK (PLAB) | |
|---|---|---|
| Key exams | Step 1, Step 2 CK (Step 3 later) | PLAB 1, PLAB 2 |
| English test | OET Medicine (for ECFMG) | IELTS Academic or OET |
| Main hurdle | Residency match | Jobs and specialty training competition |
| Typical cost | High (exams, applications, US experience) | Moderate to high (exams, travel to the UK) |
| School check | WDOMS with ECFMG note | GMC-acceptable qualification |
Planning from medical school
- Before enrolling: check WDOMS and the GMC list for your chosen university. A school that does not meet these rules closes these doors.
- Years 1-2: build very strong basic sciences. Anatomy tools such as Kenhub help. Step 1 rewards deep understanding.
- Years 3-4: start a question bank. Keep notes organised for long-term use.
- Clinical years: aim for strong clinical skills and good references.
- Keep India open: many students prepare for the FMGE as well, as a safety net. The syllabi overlap substantially in basic sciences.
Classic review books are useful for revision; compare editions for First Aid for the USMLE Step 1 before buying.
Frequently asked questions
Can I take the USMLE after MBBS abroad?
Yes, if your medical school is listed in the World Directory of Medical Schools with an ECFMG note and meets ECFMG’s current eligibility requirements.
Which is easier, USMLE or PLAB?
PLAB is generally shorter and cheaper. The USMLE route is longer, but the US offers large numbers of residency positions. Both are competitive.
What IELTS score do I need for PLAB?
Usually IELTS Academic 7.5 overall with at least 7.0 in each component, or OET Medicine grade B in each part. Check the GMC website for current rules.
Can I prepare for FMGE and USMLE together?
Yes. The basic sciences overlap a lot. Many students prepare for both, keeping the FMGE as a safety net.
